In order for each child to be eligible to play during the season, the following items will need to be submitted to the team manager in advance of the season’s first practice.

• Medical Release Form (notarized) 
• Birth Certificate - Xerox copy of an official State certificate with a seal (no hospital certificates)
• 1 inch x 1 inch head shot photo of your child for Player Card
• Parent Contract
• Player Contract
