U13-U14 Select Player Financial Contract-- required to be turned in by June 1, 2009, to your team manager.

   U13-U14 Select Player Payment Vouchers--to be used when mailing in payments.

U15-U18 Select Player Financial Contract-- required to be turned in by June 1, 2009, to your team manager.

   U15-U18 Select Player Payment Vouchers--to be used when mailing in payments.

Select Players Off-Field Code of Conduct  (original signed and submitted to Compliance Officer--copy kept by team manager)

SCYSA Medical Release (must be notarized--to be given to team manager at Team Meeting)

Optional: 

• Financial Aid Application  (must be received by 6/1/09)  (Word format)  (PDF format)
• Schedule of Fee Payments for all ages
